What is Radiesse®?
Radiesse is an FDA-approved injectable made of calcium hydroxylapatite (CaHA) microspheres in a smooth gel. It works in two ways:
Instant support: the gel adds structure and volume right away.

Most people think of Radiesse as a filler for cheekbones and jawlines. It can be. But that is only one of the things it does. When we mix Radiesse with more saline and lidocaine, it stops adding volume and starts working as a biostimulator. That means it signals your skin to build its own collagen and elastin, so skin gets thicker, firmer and smoother over the following months.

How Radiesse Builds Collagen and Elastin
Radiesse is made of tiny calcium hydroxylapatite (CaHA) microspheres in a smooth gel, and those microspheres act as a scaffold that switches on your own skin-building cells.
Here is what happens after treatment:
Right away: The gel gives gentle support. With undiluted Radiesse, this is the instant lift you see on the cheekbones or jawline.
Over the next weeks: The gel is slowly absorbed. Your fibroblasts, the cells that make collagen and elastin, attach to the microspheres and get to work.
Over the following months: New collagen and elastin form around the microspheres. Studies show early collagen type III that matures into strong collagen type I, plus new blood vessels that improve skin quality.
A 2023 research review in Aesthetic Surgery Journal summed it up: Radiesse treatment increases collagen, elastin, blood supply and fibroblast numbers in the skin (Aguilera et al., 2023). Researchers also note that Radiesse does this with very little inflammation, which is part of why results look natural.
Radiesse and Sculptra both build collagen, but the research so far shows Radiesse also builds more elastin, the protein that gives skin its bounce and snap-back.
Collagen makes skin firm. Elastin makes it springy. Skin needs both to look youthful, especially on the neck, décolletage and areas with crepey texture.
Radiesse vs. Sculptra: The Elastin Difference
In a split-arm study comparing the two, both treatments produced similar amounts of new collagen, but skin treated with Radiesse showed more elastin (Aguilera et al., 2023, reviewing Mazzuco et al., 2022). A 2025 Modern Aesthetics comparison reported similar findings (Modern Aesthetics).
To be fair, the comparison studies are small, and Sculptra remains an excellent biostimulator. We choose between them based on your skin, your goals and the area we are treating.
Collagen Type I vs. Type III: Why Both Matter as We Age
Young, healthy skin needs both types of collagen: type I for strength and type III for softness and flexibility.
Type I collagen is the strong, thick fiber. It makes up about 80 to 90% of the collagen in adult skin and gives skin its firmness (Shin et al., 2019).
Type III collagen is the finer, more flexible fiber. It is plentiful in young skin and is the first collagen your body makes when it repairs and renews tissue. Over time it matures into type I.
With age and sun exposure, skin makes less collagen of both types and breaks down more of it. Skin gets thinner, looser and less bouncy.
This is why we like that Radiesse follows the natural order: it builds type III collagen first, then type I as it matures (Aguilera et al., 2023). Add the extra elastin, and the result is skin that is firm, supple and springy, not just tight.
Inflammation vs. No Inflammation: How Each One Builds Collagen
Sculptra builds collagen through a controlled inflammatory response, while Radiesse builds it by directly signaling your collagen-making cells, with very little inflammation.
Sculptra: Its particles attract immune cells called macrophages. This low-grade, ongoing inflammation is what leads your skin to make collagen.
Radiesse: Your fibroblasts touch the microspheres and respond directly by making collagen and elastin. Researchers describe this as regeneration with minimal immune response and no chronic inflammation (Aguilera et al., 2023).
What this means for GLP-1 users. Semaglutide, tirzepatide and other GLP-1 medications have anti-inflammatory effects. Some experts suggest this could quiet the inflammation Sculptra relies on. This is still a theory, not a proven effect, and one study did find Sculptra worked in GLP-1 users (Lorenc et al., 2025). Because Radiesse does not depend on inflammation, we often see it as the more predictable choice for clients on GLP-1 medications.
What this means for inflammation-prone skin. Both products should be postponed if the treatment area has active inflammation or infection, and both carry precautions for clients with autoimmune conditions. For clients whose skin tends to overreact, we often prefer Radiesse's low-inflammation approach. Your provider will review your health history before recommending either one.
The Radiesse Dilution Guide: Structure to Pure Biostimulation
The more we dilute Radiesse, the less volume it adds and the more it works purely as a collagen and elastin builder, placed closer to the skin's surface.
Dilution means mixing Radiesse with sterile saline and lidocaine for comfort. A ratio of 1:2 means one part Radiesse to two parts liquid.
These ranges come from published protocols and expert consensus on diluted and hyperdiluted Radiesse (Massidda, 2023; Aguilera et al., 2023). Hyperdilute Radiesse is usually given as a series of treatments about a month apart. Your provider chooses the exact ratio for your skin thickness, age and goals.
